Job Description – Licensed Master Social Worker (LMSW)

Social Workers are part of a dedicated, compassionate, Hospice interdisciplinary team. They play a vital role in helping patients and families with social, emotional, and practical challenges through the end-of-life journey.
Hospice Social Workers provide information on community resources, assist with Advance Directives, and offer expertise to help problem solve and cope with caregiving and decision making.
Our Social Workers provide counseling and interventions to patients and families according to their individualized plan of care.
Hospice care primarily takes place within a patient’s home but may include residential settings such as comfort care residences or group homes.
Our Social Worker’s primarily work Monday-Friday from 8-4:30 or 8:30-5:00. If you are seeking to improve your work-life balance, with minimal after hour or weekend coverage, becoming a Hospice Social Worker will meet that need!
Candidates must excel in working with diverse communities and within a variety of settings. A valid New York drivers’ license and reliable transportation is required.

Qualifications and Skills – Licensed Master Social Worker (LMSW):
- Master’s Degree in Social Work from an accredited college is required.
- Licensed Master of Social Worker (LMSW)
- Post-Master’s experience in medical social work, grief counseling, hospice and/or community health preferred
- Excellent counseling, advocacy, and collaboration skills
- Valid NYS Driver’s License and reliable transportation
Salary Range: $30.00 - $31.50 per hour
