Stoneleigh Residence Nurse (12 Hour Shift Position)

The registered nurse plans, organizes and directs hospice care for the patients residing at Stoneleigh Residence. This nurse is responsible for overseeing the floor and providing direction to nursing assistants to ensure the patients (up to 8) at Stoneleigh Residence are given the highest quality care.

Essential Job Functions/Responsibilities

Patient Care – Hospice RN

  • Completes an initial and comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
  • Works collegially with teammates. Supports and enhances teamwork.
  • Understands the hospice levels of care and transitions patients into the various levels of care.
  • Works in cooperation with the family/caregiver and hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver.

Communication

  • Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.
  • Participates in Morning Reports and provides relevant information to daytime case managers as well as the interdisciplinary team.
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
  • Communicates appropriately with on-call administration and clinical leadership to meet the afterhours needs that may arise.
  • Teaches the patient and family/caregiver self-care techniques as appropriate.

Additional Duties

  • Assumes responsibility attendance and participation in continuing education and in-service classes.
  • Actively participates in quality assessment performance improvement teams and activities.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Supervises all personnel including ancillary staff including volunteers, hospice aides and orientees.


Position Qualifications

  • Graduate of an accredited school of nursing. One (1) to two (2) years of nursing experience in a facility setting (hospital or LTC), and one (1) to two (2) years of recent experience in hospice is preferred.
  • Current licensure in State, CPR certification and valid driver’s license.
  • Must possess a valid driver’s license and have an automobile that is insured in accordance with state law and is in good working order. Must not have more than three moving violations in the last three years.
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills; nursing skills per competency checklist.
  • Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
