Description
We are currently seeking a dedicated, compassionate Nurse-Leader to join our Team! This position is Full-time with a schedule of 40-hours per week, Monday - Friday, plus 1 'warm' holiday/1 'cold' holiday/year. Some evening and weekend hours may be required.

As a Clinical Manager, Functions as a role model in area of clinical specialty, demonstrating competency in clinical judgment, selected technical skills and appropriate knowledge base to lead and guide staff. Demonstrates clinical expertise in the execution of the nursing process and directs staff in its application to promote safe, effective, and appropriate care. You will work with your team's Case Mangers, Social Workers, Chaplains, and CNAs, as well as our Hospice Physicians, to ensure that staff is familiar with and adheres to HOC policies and procedures. We empower our Clinical Managers to serve as a resource to your staff on care continuity and quality.
Main Responsibilities
  • Serves as a marketing presence for all nursing homes and assisted living centers who partner with Hospice of the Chesapeake (HOC). In cooperation with Business Development, creates and implements opportunities for increasing facilities market share in Anne Arundel County.
  • Maintains close relationships with healthcare professionals in assigned facilities in order to maintain constant visibility of HOC programs and services and troubleshoots real or perceived problems that could impact patient referrals.
  • Develops and implements educational programs to meet the needs of the facilities staff in cooperation with Facilities Liaison/Social Worker and/or Business Development staff.
  • Functions as a role model in area of clinical specialty, demonstrating competency in clinical judgement, selected technical skills and appropriate knowledge base to lead and guide staff. Demonstrates clinical expertise in the execution of the nursing process and directs staff in its application to promote safe, effective and appropriate care.
  • Hires, supervises, evaluates and mentors staff assigned to their team, including creating and overseeing a plan of professional development for each staff member to ensure up-to-date credentials and compliance with certification and licensure requirements. Prepares and delivers staff evaluations in a timely manner. Establishes and oversees completion of individual performance improvement plans as necessary.
  • Conducts patient visits to offer support and expertise to the nursing staff, patients and families. Serves as a resource, ensuring that patient problems are addressed and resolved in a timely manner.
  • Other duties as assigned by the VP of Hospice Operations or the Chief Operating Officer.

Qualifications
Your Qualifications:
  • Bachelor's degree in nursing, healthcare administration or behavioral health required within 18 months of accepting the positions.. Graduate of an accredited nursing school required.
  • 3-5 years nursing experience in a nursing home, assisted living center, or other post-acute, long term care, or similar facility required.
  • 2-3 years supervisory experience required.
  • Excellent oral, written and interpersonal communication skills.
  • Knowledge of Medicare, Medicaid, private insurance and other community programs and resources.
  • Demonstrated ability to work effectively on an inter-disciplinary team.
  • Excellent customer service philosophy and skills required.
  • Understanding of and concurrence with the hospice philosophy of palliative care.
  • Basic computer skills, including but not limited to Microsoft Word and Outlook, are required.
