Full-time role - Monday-Friday POSITION SUMMARY: The Palliative Nurse Practitioner in collaboration with the Pathways Palliative Medical Director and the Director of Pathways Palliative Care provides advanced palliative care services to patients and their families. The Nurse Practitioner will support palliative patients with medical necessity visits on an as needed basis for symptom management, change in condition, within 7 days of hospitalization/d/c, supervisory visits over clinical team members, and assist with face-to-face visits for hospice eligibility as needed by requirements of Medicare CoPs for hospice admissions in benefit period 3 or higher. The Palliative NP will obtain medical histories, conduct physical exams, and educate patients and families in the disease process. This position requires the ability to manage administrative performance; expertise in oral and written communication, staff supervision, managing documentation and service coordination, active participation in QAPI program and active participation in pain management program with supervision, in person visits/monitoring and writing prescriptions in collaboration with Palliative Medical Director. QUALIFICATIONS: Education: Master's degree in nursing from an accredited school of nursing. Credentials: Current CA Nurse Practitioner License. Certification in Hospice and Palliative Care preferred. Experience: Minimum of 3 years of hospice and/or palliative care setting experience at an advance level preferred. Must possess sound judgment, organizational skills and the ability to prioritize and follow-through accurately and efficiently. Computer skills are required.
