Hospice of the Piedmont is Hiring a Hospice Homecare LPN Near High Point, NC

Essential Duties

  • Duties are delegated by and performed under the supervision of a registered nurse.
  • Participates in assessment of the patient’s physical and mental health, reaction to illnesses and treatment regimes.
  • Observes the patient’s condition and reports changes of condition and the needs for nursing measures requiring additional orders to the patient’s nurse case manager and or MD as directed by RN.
  • Makes nursing visits and implements nursing activities consistent with the patient’s Plan of Care as developed by the staff nurse (RN). These include administration of prescribed medical treatments, wound care, phlebotomy, medications, and basic bedside care. In addition, may irrigate, collect blood, change dressings; give medications/ fluids through peripheral, PICC, midline, central venous access, and IVADs. May change dressings and sites, give medications/ fluids through subcutaneous access catheters. May remove peripheral, PICC, midline, central venous catheter and subcutaneous catheters as assigned by RN.
  • May pronounce death.
  • Acts in a professional manner to collaborate with physicians, other health care providers, and interdisciplinary team members and makes timely reports of pertinent information to appropriate health care professionals.
  • Documents data collected for assessments and interventions in an accurate and timely manner according to the nursing standards of this agency.
  • Attends IDT Meeting per Director’s direction.
  • Delegates appropriate personal care tasks to Hospice Aides and supervises that tasks have been completed.
  • May assist in basic nursing and personal care.
  • Participates in teaching the patient and caregiver on home care techniques as assigned by the staff nurse.
  • Assists with audits as assigned.
  • Participates in the nursing on call schedule, as needed, under the supervisor or the RN on call.
  • Complies with agency nursing standards, completes timely documentation and demonstrates competency in the approved infusion therapy tasks.
  • Ability to lift, turn and transfer patients; minimum lifting requirement 40 pounds.
