The Hospital Liaison (RN) position is primarily responsible for assisting the Referrals and Admissions Department in admitting patients onto agency services including hospice, palliative care, Staywell and other programs offered by Hospice of the Piedmont. The Hospital Liaison participates in admissions, pre-assessments, and other related visits across various settings including the home, inpatient units, hospitals, long-term care facilities, or all other settings as necessary. Hospital Liaison shall assure appropriateness for services, verify orders, and communicate with other members of the interdisciplinary team to develop a plan of care.
