Hospice of the Piedmont is Hiring a Palliative Care RN Case Manager (24 hrs) Near High Point, NC

Work Schedule: 3 days/week (TBD) - 24 hours

This position supports our palliative care program, Care Connection, and demonstrates competency in delivery of quality patient care to patients ranging in age from infants to geriatrics. The RN Case Manager is responsible for developing and implementing a plan of care in accordance with the patient’s expressed wishes and delegates care and interventions in full compliance with the Nurse Practice Act and organizational policy, procedures and standards of care.

Essential Duties

  • Plan, provide, supervise and document safe, competent and compassionate hospice nursing care for patients in accordance with physician orders and established policies and procedures.
  • Develops and coordinates plan of care in collaboration with patient, significant others, and interdisciplinary care team members to focus on holistic needs of patient in order to assure satisfactory outcomes and achieve patient care goals.
  • Makes nursing visits according to the Plan of Care. Delegates appropriate interventions to LPN and nurse aides. Follows organizational policy at time of death.
  • Provides direct patient care according to the plan of care and in accordance with the Nurse Practice Act and Hospice Standards of Practice.
  • Acts in a professional manner to collaborate with physicians, other health care providers, and interdisciplinary team members and makes timely reports of pertinent information to appropriate health care professionals.
  • Accurately and timely documents nursing assessment, interventions, and updates to Plan of Care according to the nursing standards of this organization.

Experience

Preferred
  • 1 year(s): Clinical nursing experience

Education

  • Associates or better in Nursing
