Position Summary
Provides nursing care to hospice patients in coordination of the interdisciplinary team (IDG) that is consistent with the goals, definitions, philosophy, and purpose of Hospice of the Red River Valley (HRRV). Nursing care, as approved by patients' primary physicians or the hospice medical director, including physical assessments, symptom management, and emotional support.
This position will be full-time, benefit eligible, and will be based in the Lake of the Woods/northwestern MN service area.
Key Responsibilities

• Gather and report data on the patients/family's physical, social, and emotional needs on an ongoing basis while hospice care is being provided to the patient, under the direction of a registered nurse.
• Provide nursing visits to patient/family as determined in the plan of care.
• Teaches and supports patient/family in areas related to the patient care and preparation for death as directed by the Registered Nurse.
• Accurately documents all patient/family/physician contacts and interventions in a timely manner.
• Maintains communication with the Registered Nurse regarding the patients changes in status.
• Completes and locks documentation on the same day as the contact/communication occurs as per the Medical Records Documentation policy.

Qualifications
Qualifications
LPN degree
Minimum of one year of experience as a hospital staff nurse and/or minimum of two years of experience in health care.
Strong interpersonal communication, counseling, and teaching skills
Flexibility in scheduling; team player
Familiarity with principles related to dying and death strongly preferred
Certifications, Licenses, Registrations Required
Current state LPN licensure in North Dakota and Minnesota (or willing to obtain dual licensure upon hire)
Current driver's license and auto insurance
