FUNCTION:
The Patient Care Coordinator (PCC) is responsible for the coordination of all aspects of department operations. The PCC has primary responsibility and authority for maintaining quality of patient care, in accordance with the organization’s objectives and policies through planning, coordinating, implementing and evaluating the services provided patients and families.
SUPERVISION RECEIVED:
Reports to the CEO.

Nurses, Hospice Aides & Clerical staff of assigned department
EXAMPLES OF WORK PERFORMED:

Job Summary:
· Coordinates and oversees all direct and indirect patient services provided by department clinical personnel
· Provides guidance and counseling to department clinical staff to assist them in continually improving all aspects of hospice care services
· Assists clinical staff in the coordination of services between Home Care and Rainey Hospice House
· Develops and maintains a staffing schedule to ensure 24/7 coverage of clinical staff within assigned department
· Provides assistance in assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated.
· Assists the CEO and CFO in the preparation and administration of the organization’s budget
· Interprets operational indicators to detect census changes and increases or decreases in volume that could impact staffing levels, revenues or expenses and schedules staff accordingly
· Evaluates performance of nurses, nursing assistants, clerical staff and other clinical personnel
· Assists clinical staff in developing and increasing skills and techniques in evaluating the patient and family situations
· Coordinates Interdisciplinary Team Meetings at least every 15 days and sensures communication with the team if warranted by a patient’s change in status.
· Interviews, hires and trains direct reports
· Regularly reviews patient clinical records, statistics and reports for purposes of program evaluation
· Assists Medical Director with coordination of care to ensure regulatory compliance
· Assures proper maintenance of clinical records in compliance with local, state and federal laws
· Responsible for the adequate and appropriate coordination of supplies and equipment for the provision of patient services
· Responsible for planning and implementing either directly or indirectly, the clinical orientation of new personnel
· Participates with the QM and Education Manager in planning and implementing in-service and continuing education programs to meet education and training needs of staff
· Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews. Assures for the quality and safe delivery of hospice services
· Assists in the development of organization and department goals and assures that the organization’s policies and procedures are implemented and followed
· Assures compliance with all local, state and federal laws regarding licensure and certification of organizational personnel and maintains compliance to Hospice of the Upstate standards
· Stays informed about changes in the field of nursing and hospice care; shares information with appropriate organization personnel
· Represents department and Hospice of the Upstate as an ambassador to internal and external customers and vendors
· Promotes hospice refrrals in the health care community
· Ensures the protection of patient and employee information by following HIPAA-HITECH guidelines
· Assures the employees maintain the integrity of all forms of patient information, including but not limited to electronic personal health information, assuring that there are no attempts to access more than the minimum necessary information to complete his/her job
· Be organized, flexible and work effectively and professionally under stress and deadline pressures
· Professionally represents the organization with credibility and dignity in the spirit of excellence and trust
· Demonstrates and communicates the mission, ethics and goals of the facility and “the face of Hospice of the Upstate”
· Other duties as assigned by the CEO
DESIRED MINIMUM QUALIFICATIONS
· Registered nurse in the State of SC with current licensure to practice professional nursing in the State of SC
· Bachelor’s degree in Nursing from an accredited program by National League for Nursing
· Management experience in a hospice or related health care organization preferred
· Demonstrated ability to supervise and direct professional and administrative personnel
· Ability to market and deal tactfully with customers and the community
· Has excellent observation, verbal and written communication skills
· Knowledge of business management, governmental regulations and Medicare Conditions of Participation (CoP) standards
· Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order

PHYSICAL DEMANDS:
May be required to climb steps, stoop, bend or lift up to 50 pounds. Must be able to communicate compassionately with patients and families. Must be able to stand for long periods of time.
