At Houston Methodist, the Population Health Advisor position is responsible for understanding the programs and initiatives being deployed by Houston Methodist Coordinated Care, identifying practice improvement opportunities and integrating clinical programs and resources into the practice workflows with the intent to optimize the patient experience, quality of care provided, and positively impact resource utilization.
This position supports the physician practices by analyzing performance reports and data to inform decision-making, process, and program implementation.
The Population Health Advisor position monitors and implements educational and process interventions based on practice-level data, trends and identified opportunities.
