Human Service Agency is Hiring a Direct Support Coordinator - Nutrition Near Watertown, SD

DESCRIPTION OF POSITION:
The mission of New Horizons is to support people to achieve self-reliance, exercise control over their lives and realize their dreams and desires. The Direct Support Coordinator - Nutrition is responsible to guide, counsel, and provide supports for people. To support them to live the life they desire by achieving their goals, dreams & desires. While providing protections for their health, safety and welfare. This position is also responsible for supervising staff that will be providing supports to people. This position is responsible to assist each person supported to achieve personal outcomes that reflect their dreams, hopes, and desires.
Position Details:
Full Time: 40 Hours per Week (Monday - Friday).
Shift: Days
Job Knowledge and Comprehension:
A. Provide oversight and supervision
1. Hire, train, and provide coaching and mentoring to department staff.
2. Conduct routine supervisory meetings, approve time cards, approve schedules, and ensure efficient use of staffing patterns are in place within department budget.
3. Complete supervisory paperwork and documentation in an effective and efficient manner.
4. Maintain supervisory note files and conduct performance evaluations following agency practices. Handle disciplinary issues as they arise, and ensure personnel policy and procedures are followed and enforced.
5. Serve as an advocate for agency regarding policies, practices, and changes and a resource to keep your staff informed of agency happenings.
6. Oversee the staffing program for department. Assess on-call program and determine whether changes need to be made based on agency need.
7. Review and approve all leave requests, time-sheets, and required reports within deadlines.
B. Staff is knowledgeable of assessment and processes used to discover the needs, preferences, and capabilities of the persons supported and how that information is used in development and review of the service plan.
C. Monitors crisis situations, discusses incident with authorized staff and people supported. Adjusts supports to individual preferences and helps maintain a safe environment and complies with regulations for reporting.
D. Keeps the Vocational Services Manager informed of significant problems and errors in the department, their causes and steps being taken to correct them.
E. Maintains security and safety of People we support.
F. Performs other duties as assigned consistent with exempt functions.
Work Quality and Accuracy:
A. Maintains standards of confidentiality and ethical practice.
B. Uses effective, sensitive communication skills to build rapport and channels of communication by recognizing and adapting to the range of individual communication styles.
C. Assists Vocational Services Manager in planning and implementing department budget.
D. Completes necessary financial and statistical reports for Administration.
E. Performs cost analysis of supply items and obtains price quotations and information on products from vendors.
F. All duties relative to inventory control and cost accounting of supplies for the department.
G. Ensures that all assets assigned to the department are kept in good condition and in a state of good repair.
H. Ensures that all purchasing within the department is done in a cost-effective manner (i.e. the best quality for the best price).
I. Ensures that every aspect of the department is in compliance with all applicable laws, regulations and standards.
J. Monitors and reviews all planning, forecasting and projections relative to equipment, space and staffing services.
K. Plans cycle menus for group homes and institutional kitchen to meet the nutritional needs of individuals set by accrediting agencies.
L. Develops, implements and adheres to nutrition evaluations to better screen the individuals for nutritional deficiencies.
M. Develops, implements and supervises the use of production and cleaning schedules and recipes to meet all service needs.
N. Supervises and directs the preparation of food prepared in the institutional kitchen and group homes assuring that approved production and service guidelines are maintained.
O. Develops and implements in-services or teaching necessary to better the health of the individual served, i.e., weight loss, portion control, low-fat microwave cooking.
Physical Requirements:
Lifting requirements may exceed 50 lbs. during medical emergency situations and/or behavior support. Other physical activities include transferring people we support from and to wheelchairs, beds, and toilets.
Job requires bending, stooping, climbing stairs, pushing/pulling, and driving a vehicle.
Degree in nutrition/food science or certification as a dietary manager
Minimum of two years experience in food service
Valid drivers license and safe record
Successful completion of criminal background check
The Board of Directors of the Human Service Agency shall have the prerogative to hire staff whose qualifications may vary from the above. This position requires access to Electronic Personal Health Information (EPHI). Current HSA Policies and Procedures apply.
