Reporting to the Director of Developmental Disabilities Services, the Community Integration Facilitator is responsible for facilitating and actively participating in Person Centered Planning for adults with Intellectual/Development Disabilities while collaborating with team members to provide clients with meaningful workshops/modules on a variety of topics including but not limited to life skills, social skills training, and vocational and recreational activities. This role combines direct support/group facilitation with case management responsibilities and plays a key role in developing and setting priorities and objectives to align with treatment goals. This role will require some transportation of clients via personal or company vehicles.
