Job Title: Restorative CNA

Primary Function and Relationship to Total Organization

The Restorative CNA will safely and effectively provide rehabilitative therapies under the direction of a Physical Therapist or Occupational Therapist. Assists nurses in providing nursing care. Works under the direct supervision of an RN.

Reports to: Restorative/Quality Nurse and/or Director of Nursing

Essential Duties and Responsibilities

1. Provides rehabilitative/restorative therapies and exercises to residents according to their plan of care.
2. Works under the guidance of PT/OT to assure the therapeutic care plan is being followed and carried out.
3. Assists with bathing, feeding, and caring of residents according to the level of skill and as directed by the nurse.

1. Reports all significant information to the nurse in charge of the resident.
2. Works with other health personnel within the organization.
3. Does cleaning of resident units, equipment, and nursing service areas.
4. Keeps charge nurse informed on care rendered and duties performed.
5. Answers call lights and alarms promptly.

1. Delivers residents trays, assists in feeding residents when necessary, records intake and outputs and returns trays to the kitchen.

1. Gives verbal report to the charge nurse at the end of shift.
2. Takes TPR’s and assists with weighing residents as assigned.

1. Assists with transferring residents by wheelchair or cart to other departments. Uses an assisted lift device such as a gaitbelt or mechanical lift as indicated by the care plan. Participated as a member of a safe lifting team.

1. Complies with hospital and nursing service policies.
2. Attends staff meeting and in-service education when appropriate.
3. Demonstrates willingness to support staffing and scheduling needs.
4. Performs functions other than described above due to extenuating circumstances.
