Husky Senior Care is Hiring a Care Manager Near Seattle, WA

Job Overview
The Care Manager works with Husky’s clients to achieve the goals determined by each client’s plan of care. Care Managers conduct in-depth assessments, develop care plans and recommendations, coordinate services, act as liaisons to health care providers and insurers, and continuously monitor services to ensure that the client’s goals are met. They are knowledgeable about the wide array of resources available.

Duties
o Managing approximately 15-20 clients at a time.

o Make sure that each customer’s Plan of Care is up-to-date, adequate to serve their needs, and reflects that care provided by our caregivers in the home.

o Stay up to date on client care through a combination of in-home visit, reading weekly reports, and caregiver feedback.

o Triage needs so that heavier care clients receive extra attention.

o Communicate with the client’s contact(s) to lower anxiety and give them confidence in our care.

o Perform inspections and loop that back into caregiver and Plan of Care development to insure excellent care.

o Problem solve in special client care circumstances, including change in condition, emergency start-up, discharge from hospitals, hospice start-up, and setting up nurse delegation.

o Improve medication tracking, introduce bed alarms and other tools, work with caregiver(s) to fine-tune care.

o Communicate well with client’s contacts during stressful times.

o Review weekly reports.

Requirements:

  • Excellent English skills, both speaking and writing.
  • Proficient in Word and Excel.
  • Criminal, Credit, Credential, and Reference Checks.
  • Caregiver Certification, CPR and First Aid certification.
  • Willingness to travel to client homes by personal car and transport clients.
  • Must be comfortable with pets (cats, dogs, etc.).
  • Must be able to lift at least 50lbs by self.
  • Sincere desire to help people live better lives, especially senior citizens.
  • Extremely high personal integrity and reliability.
