Purpose: I-2-ISolutions has the goal to provide exceptional student (special education)

services by filling the vacant contract position Certified Occupational Therapy

Assistance at Crownpoint Community School (CCS) for the fiscal school year

2024-2025. Estimated dates of July 1, 2024-June 30, 2025, includes 4 weeks ofESY in

the month of June 2025.

Methodology: Certified Occupational Therapy Assistance in person services will be

delivered in a culturally competent manner. The specific methodology incorporates a

in person-centered approach which will provide empowerment to CCS students. The primary

purpose is to focus on building effective relationships with students, educators,

administrators, and parents of CSS. The service providers will facilitate and support each

CCS student to reach their highest educational potential in becoming productive

members of a multicultural society.

The Scale of Therapy Services:

Occupational Therapv: A licensed Occupational Therapist assistant (OTA) is to provide

occupational services to 6 identified students in the CCS Special Education department.

• The occupational therapist assistant will work onsite one to two days per week,

throughout the fiscal school year, to ensure these identified students receive adequate

OT counseling services as mandated on their individualized education plans.

o An OTA will evaluate and interpret evaluation results for the IEP committee.

• OT A interventions will be designed and implemented appropriately to meet the needs

of the individual students as determined by the IEP committee.

• The OTA will provide teacher consultation services.

• The OTA will provide daily performance data, assist with goal writing, participate in

IEP meetings for students on caseload, keep an active roster of students, submit case

logs, and submit progress reports promptly, provide evidence of services through

documentation of service log completion at the end of each session to be filed

appropriately in NASIS. Contractor shall sign-in and sign-out at the front office .

Plan for Providing Services: To provide quality services, the licensed service providers will

conduct the following:

Therapy:

• Conduct occupational Therapy to 6 identified students.

Assessments:

• Conduct comprehensive occupational Therapy evaluations to assist the exceptional

education department in whether a child meets or still meets the requirements for an

exceptional educational placement.

o The assessment tools to be used are standardized and norm-referenced.

• Review and analyze current evaluation reports

• Review student medical and educational records

• Evaluations are required to be completed within 60 days as mandated by IDEA.

• Participate in Multidisciplinary Evaluation Team meetings to present evaluation results to

parents, teachers, and adininistrators professionally and meaningfully. Additionally, to

make further recommendations to benefit the student.

• Attend IEP/MET meetings to discuss evaluation results and recommendations

- Develop IEP goals

- Input NASIS goals and service log in NASIS

- Staff consultation for addressing academic, behavioral, occupational, physical,

and speech concerns.

• Occupational therapy for 6 identified special education students

Progress Monitor:

• Must submit quarterly progress notes and therapy logs for students promptly.

• Will sign in and sign out verifying attendance at school.

• Must provide their own personal equipment (laptops, kajeets, printers, supplies/

materials, curriculum materials)
