Job Overview:

We are seeking a dedicated Field Registered Nurse to join our healthcare team. The Field Nurse will be responsible for providing high-quality patient care in the patient's residence. We provide a flexible schedule for our field staff, it will be up to the Field RN to schedule with the patient when they can be seen according to the Plan of Care. We reimburse for mileage after 25 miles at $0.78/mile.

- San Bernardino County

- Riverside County

- Los Angeles County

Responsibilities:

- Admit patient into home health service

- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor and report changes in patient symptoms or behavior
- Educate patients and their families on proper healthcare procedures
- Collaborate with physicians and other healthcare professionals to ensure optimal patient outcomes
- Maintain accurate and up-to-date medical records

Requirements:

- Bachelor's degree in Nursing or related field
- Valid California RN license
- Previous experience in standards of patient care
- Proficiency in laboratory procedures and protocols
- Familiarity with HIPAA regulations
- Experience in hospice palliative care is a plus
