PURPOSE OF POSITION: Provide high-quality direct services at the center(s) in the areas of health, disability, safety, and nutrition. Coordinate and carry out direct comprehensive health services for children and families that include a range of medical, dental, and nutritional services and follow-up in accordance with goals and federal performance standards. Work hands-on with children and families and therefore, must be able to communicate and respond in an effective manner that consistently demonstrates respect and concern.

ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following; however, other duties may be assigned:

1. Arrange and conduct the hearing, vision and dental screenings on all enrolled children; or coordinate with local resources to conduct these screenings, assisting in their screening process as needed. Follow-ups after the screenings are done in collaboration with center team: 1) Share all results with families. 2) Ensure children needing dental exams and follow-up treatment or vision/hearing get services that are documented in child’s file/electronic record. 3) Complete and provide families with referrals and consistent follow-up. Follow standards and timeframes listed in RCMA Plan of Action.

2. Ensure, in collaboration with center team, that all children stay current on:

a. Immunization status, and utilize information available through Florida SHOTS.

b. Well Child Check-ups following the Florida Medicaid (EPSDT) periodicity schedule.

c. All other health screenings are completed and documented (blood lead and hemoglobin/hematocrit).

d. Review the Health & Nutrition Assessment from MyHeadStart and provide families with referrals and consistent follow-up.

3. Assist center team, when necessary, in making referrals for disability services to Early Steps, FDLRS, or other agencies. Coordinate the teamwork necessary to complete the disabilities summary sheet and review for accuracy prior to submitting to the Health, Safety, and Nutrition Specialist on a monthly basis. Work in collaboration with families and the family’s FSW to arrange services including providing or arranging transportation as needed for any health and/or disability-related appointment.

4. Work in collaboration with IDS to make sure all health and disability-related data and follow-ups are entered in our data system MyHeadStart. Review and analyze data, corrections as needed are made using MyHeadStart reports. Ensure the Health & Disabilities PIR data is accurate.

5. Collaborate with FSW to provide information, education, and services to enrolled pregnant women and family members of children enrolled, encouraging a healthy pregnancy and postpartum care, conducting newborn home visits (birth outcome), offer services, plan transition to program enrollment, etc. Use Partners for a Healthy Baby Curriculum. Work with Healthy Start or Healthy Families

6. Complete tasks listed on the Health Worker Checklist (H24) which includes child file documentation, classroom observations, and disability services, e.g. reviewing teachers’ Daily Health Checks and medication documentation. Conduct the Health & Safety Checklist 2-3 times per year/per center, depending on the length of center program year. Coordinate Center safety committee with Center Coordinators. Maintain safety standards in accordance with RCMA policies, state and federal regulations.

7. In collaboration with Health Specialist, Inclusion Specialist & Mental Health Manager, initiate, complete and renew health and disability agreements/contracts.

8. Use RCMA Health Book to provide guidance to classroom staff and in making child health referrals.

9. Complete 3 USDA meal reviews (breakfast, lunch and snack) per center, per season (2 reviews for MHS centers). Ensure menus are posted, dated, and followed. Monitor and support the Center Coordinator in completing all USDA required forms for all programs.

10. Engage in regular meetings with the Core Team and teachers as it relates to assigned duties. Provide training as needed. Maintain communication and consultation with the regional team. Maintain effective communication with outside agencies. Attend community meetings as it relates to role. Attend HSAC meetings as directed.

11. Ensure center(s) meet compliance with Head Start Program Performance Standards, USDA, Plan of Action, RCMA policy, DCF Licensing, local, state and federal regulations including timeframes on all assigned duties.

Education and Experience:

1. Associate Degree and/or a minimum of 5-10 years of verifiable experience in nursing, health education, community health or related field.

2. Obtain the NHSA Basic Health Services Credential by the end of 2nd program year.

3. Experience in social services or health work, some of which has been with economically disadvantaged people and children, preferably in a community setting.

4. Ability to conduct training sessions.

5. Ability to conduct health screenings using screening equipment.

6. Ability to work with paraprofessional staff, families and professionals (service providers, local education agencies).

7. Ability to work in a multicultural environment.

8. Knowledge of Microsoft Office programs such as Word, PowerPoint, and Excel.

9. Valid Florida Driver’s License and dependable transportation.

DESIRED QUALIFICATIONS: Member of community being served and/or farmworker background. Possess or willing to obtain certification in Cardiopulmonary Resuscitation (CPR) and Red Cross First-Aid.
