Job Title: Home Health Nurse

Responsibilities:

• Develop and deliver comprehensive Core A & B training programs to Home and Community Support Professionals
• Collaborate with subject matter experts to ensure training materials are up-to-date and aligned with current healthcare guidelines and best practices.
• Conduct comprehensive assessments of patients' health status and develop individualized care plans based on assessment findings.
• Monitor patients' progress and condition, and revise care plans as necessary in collaboration with the patient's physician.
• Ensure compliance with regulatory requirements, company policies, and best practices in home health care.
• Perform regular visits to assess patient needs, provide treatments, and evaluate outcomes.

Qualifications:

• Registered Nurse (RN) with a current valid license in Indiana.
• CPR certification
• Proven experience in healthcare education or training, particularly in Core A & B training.
• Proficiency in using educational technologies and multimedia tools for training purposes.
• Experience in curriculum development and instructional design.
• Minimum of 2 years of nursing experience, preferably in home health care or a related setting.
• Strong clinical assessment and critical thinking skills.
• Ability to work independently and manage time effectively.
• Excellent communication and interpersonal skills.
• Compassionate and empathetic demeanor with a strong commitment to patient-centered care.
• Reliable transportation and willingness to travel.

Job Type: Full-time

Pay: From $58,000.00 per year
