Job Summary: As a Home Health RN, responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy and standards of community health practice. Also required to perform admission processes and case management responsibilities in accordance with this client's policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.

APPLICANTS MUST HAVE:

• 1 year Home Care experience
• 1 year Case Management Experience
• Current licensure in the state of Massachusetts
• Must have the ability to travel to cases as assigned.
• Reliable, independent means of transportation to make home visits.

Responsibilities:

• Providing clinical services to patients from birth to over 100 years in their homes. Assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.
