DAYS: Work schedule consists of Monday through Friday 8-5,
with a rotation to include Saturdays approximately every fourth week,
allowing for a day off during the week to facilitate the rotation.

- Experience with hospice case management required
- Experience providing hospice care in a home setting required
- Experience with McKesson/Netsmart EMR preferred
