Yonkers Gardens Center for Nursing and Rehabilitation is now hiringan experienced MDS Assessor who is committed to providing the best life and care for our residents. Under the: direction of the Director of Nursing Services, the MDS / Care Plan Coordinator is responsible for implementing the federally mandated assessment tool to identify needs / concerns of the resident and to coordinate a plan of rare to assist the resident toward optimal wellness based on the highest practicable level of functioning.

MDS ASSESSOR QUALIFICATIONS:

• Organizational Skills/Ability to Multitask
• Knowledge of PDPM/Clinical Concepts
• Ability to work with facility nursing management to identify and fix care documentation notes
