We are looking for RN Care Manager - ED at New York, NY. Please find the job details:

Title: Care Manager ED
Loation: New York, NY 10004
Shift: 9-5 Hybrid, 40 hrs a week
Contract: 13 weeks with possible extension

Required:
Bachelor’s Degree required; BSN preferred.
Two (2) to three (3) years clinical experience in certified home health agency (CHHA), Lombardi program and/or MLTC or MAP program

Skills:

• Five years minimum of clinical experience as a Registered Nurse.
• Leadership experience preferred. Experience with Care Coordination, Utilization review and discharge planning Computer literacy required.

Education:

1. RN Licensed as a Registered Nurse in the State of New York required.

2. Baccalaureate degree required.

3. PRI and Screen certification within three months of employment is required.

Master’s Degree and Bilingual Spanish Certification in Case Management preferred.

Pay: $2,000.00 - $2,150.00 per week
