RN Case Manager
- In the field all day every day
- Goes to office 1X a week for team meeting other than that on the road
- Oversee 15-20 patients/ caseload
- Benchmark is 5 patients a day
o Outside of the day they go into the office they will probably only see 2
- Entering homes, nursing homes, assisted livings, wherever the patient lives
- Making sure care of plan is appropriate
- Seeing all patients 1X a week at least
Covering the New London/Middlesex County
