Job Description:

We are seeking an experienced Nephrology Nurse Practitioner to join the U.S. Renal Care team in Hilo and Kona Hawaii, specializing in end-stage kidney disease (ESKD) care. As a Nephrology Nurse Practitioner, you will play a crucial role in case management for patients with kidney disease, and providing comprehensive assessments, education, and care. Additionally, The NP will collaborate with the nephrologists and the care team to complete rounding in the dialysis centers alongside the care team monthly.

Must Haves:

• Current specialty certifications and license to practice as an Advanced Practitioner in the state of Hawaii.
• Firm understanding of the protocols, policies, and operations of a physician's practice
• Case Management Experience
• Minimum of 2 years of NP experience
• Experience working in Nephrology and/or internal medicine specialties
• Excellent written and verbal communication skills
• Leadership skills and ability to present to team members and upper management
• Positive and charismatic attitude

Plusses:

• Familiarity with Risk Adjustment in Value-Based Care
• Previous leadership experience

Day-to-Day:

The candidate will be working remotely the majority of the time. Responsibilities are listed below.

• Review patient charts for Chronic Kidney Disease (CKD) and make suggestions as needed
• Asisst in Referrals to IDT teams (referring case management or social worker follow-ups)
• Suggest Recommendations in EMR to the provider (pharm or care-related)
• Running interdisciplinary meetings to review patient profiles weekly
• Partnering with the nurses for case management and patient care such as:
• Follow-up calls
• Reviewing and creating discharge plans
• Standby assistant for Hawaii calls for on-demand counsel to assist in avoiding ED admissions for patients

In Addition, The Nephrology Nurse practitioner will also be completing rounding to meet and treat patients once a week for two Hawaii facilities assigned each month. The NP will also be responsible for cultivating relationships with the physicians, care team, and patients.
