Job Description
MAJOR FUNCTION:
To facilitate and assist with the care coordination of behavioral health patients on both acute medical floors and in the Emergency departments. To collaborate amongst the integrated care team to enhance planning of clinical and social services of identified behavioral patients throughout all services lines. Provides expert feedback to the healthcare team on areas where staff may need additional support and education. Assists the team in preparing care plans and managing behaviors to optimize health outcomes.
QUALIFICATIONS:
Education & Experience:
•Master's degree in behavioral health field required.
•Welle Training required.
•Demonstrates the ability to work individually and as a team member with other clinical staff.
•Must possess the ability to work independently to accomplish objectives and use time effectively to complete projects by scheduled timeline.
•Must be organized, self-directed, and detail oriented
•Must possess the ability to retain, build upon and apply knowledge and experience gained through work-related interactions and assignments.
•Must have excellent verbal and written communication skills, can present to all levels of the organization.
•Ability to influence leaders and effectively negotiate with internal and external business partners required.
•Minimum of three years of working in a behavioral health agency working with adults, children, and adolescents.
•Must have prior experience in care navigation for behavioral health patients.
Certifications/ Licensure:
•LSW / LCSW preferred.
