- Job Description
- Are you getting burnt out seeing 4 or more patients an hour in the clinic? This may be a great opportunity for you.
- Option to work 4 or 5 days
- This is a full-time physician role caring for Medicare and Medicaid geriatric patients in the home setting.
- Our goal is to see patients with realistic time, to provide the right level of care to make the clinical impacts outlined below:
- The Physician Team Leader (Pod Leader) acts as a resource/coach impacting clinical performance of the team. The leader will collaborate with advance practice providers and build a collaborative team.
- Success in Value Based Care changes the demands placed on primary care. We are investing in physicians to do what they do best: provide outstanding care to complex patients with a complete care team.
- What to expect as a Physician Pod Leader:
- Time and resources to focus on your complex patients
- Opportunities to teach and mentor
- Clinical supervision of advanced practice clinicians
- Interdisciplinary care collaboration
- Interaction with medical leadership
- Primary Care Practice - As it should be!

- More quality, face-to-face interaction with patients and families
- No weekends, No holidays

- Board Certified Family Medicine, Internal Medicine or Geriatric Medicine
