What will you be doing:
As a Case Coordinator, you coordinate medical services for home health patients with direct oversight of the Case Manager, RN ensuring positive patient experiences; liaison between our company and our patients; verify patient information, troubleshoots equipment to resolve patient’s issues, provide delivery information, responds to health plan inquiries, process -re-authorizations, escalate unresolved issues, complete and process any pending orders. The incumbent will take ownership ensuring the patient obtains coordinated medical services effectively solving any issues, complaints and inquiries; keeping customer satisfaction and medical necessity at the core of every decision and behavior in recommendation of the Registered Nurse, Case Manager.
