Purpose of Position:
Plans, implements and evaluates nursing care, utilizing nursing process for assigned patients. Provides for psychosocial needs of patients.
Job specific standards:
Assessment:
• Performs body system assessment and accurately and promptly, including assessment of pain
• Assesses patient/family educational needs and expectations of hospitalizations
• Incorporates clinical indicators (labs, radiology, vital signs) in SBAR hand-off report to physician or other staff
• Assesses the biological, psychological, physical and cultural needs of patients/families
• Recognizes and provides ongoing assessment of any change in patient condition
Collaboration:
• Participates with all disciplines in providing for assessed needs of assigned patients.
• Is accessible to unit at all times and Clinical Supervisor is always aware of location.
• Willing to ask for help when necessary.
• Participates in unit-based problem solving.
• Attends 75% of all staff meetings.
Collegiality:
• Oversees and assigns direct reports within their Product of practice. Assist direct reports with problem solving
• Shows autonomy in conversing with physicians and physician assistants in every interaction
• Contributes to a healthy and supportive work environment.
• Communicates effectively with staff
Diagnosis:
• Monitors, recognizes and reports abnormal test results
Education:
• Provides health education and procedural information to patients and families based on assessed needs
Ethics:
• Follows the chain of command and serves as the patient advocate within applicable laws and patient rights.
Evaluation:
• Considers patient views about healthcare and documents compliance or lack of willingness to make life style changes
Implementation:
• Implements the individual interventions from the plan of care
• Identifies possibilities for pos-hospital care from outside agencies
Leadership:
• Performs relief charge role when necessary after orientation
Planning:
• Develops and evaluates plan of care based on assessed needs of the patient and family. Initiates and updates the plan to meet the ongoing needs of the patient and family
• Ensures the workplace is clean and ready for anticipated activities.
Professional Practice:
• Audits/tracers show minimal documentation errors
• Serves as a resource to others. Preceptor to students and new hires
Quality of Practice:
• Washes hands prior to each contact and use gloves when any soiling may occur
• Documents appropriately for transfer of patient care inter-and intra agency. May need assistance with COBRA/EMTLA guidelines.
• Demonstrates appropriate clinical skills.
• Provides care independently to assigned patients
• Demonstrates skills in using special equipment such as monitors, infusion pumps, or oxygen equipment.
• Demonstrates compliance with quality measures.
• Documentation is accurate, timely, and problem-focused
Resource Utilization:
• Exhibits sound time management skills to prioritize care for patients. Manages time effectively to minimize overtime and burden to next shift.
• BLS - Required
• PSYCH/BH experience - Required: 2 Years
• RN License - Required
• Travel Experience - Required: 1 Years
Education:
• Graduate of an accredited or NLN-approved RN program – Required
• BSN - Highly desired
