Clinical/Client Liaison

We need a seasoned, licensed, professional nurse, social worker or experienced liaison who understands the healthcare continuum.

As a Clinical/Client Liaison for Integritus Healthcare, you will be an integral component of how we provide essential and needed services to those individuals needing short-term rehabilitation or long-term skilled care. You will be our point person for coordinating and delivering on critical aspects of how our referred patients are reviewed and approved for placement in our affiliates. Your excellent computer, communication, and organizational skills will be critical to completing clinical screens, PASRRs, document uploads from hospitals, and making field-based decisions for admissions before the resident transfers.

Responsibilities:

  • Completes essential clinical referral screening for new referrals from acute care settings.
  • Completes clinical screens, PASRRs, or bed-hold updates as requested.
  • Manages case manager relationships at referring acute care hospitals via electronic and in-person methods.
  • Connects with families and patients, where possible, to provide specific information about services to assist patient in decision making process.
  • Supports and executes client/clinical liaison duties as determined by department and organizational leadership.
  • Verifies payor source for all accepted referrals scheduled for admission providing referral manager with any payor-related issues or concerns.
  • Functions as system administrator for the various electronic referral systems within the market assigned.
  • Monitors electronic referral systems for updated documents and attaches clinical documents as necessary.
  • Works collaboratively and cohesively with the various teams, including facility-based and Liaison teams, to understand 5 Star and Service mix goals and assist with ideas and plans supporting the common objectives.
  • Understands and works in support of the organization's mission, vision, and values, always conveying a positive image of Integritus Healthcare and its facilities to all audiences

Qualifications:

  • Successful experience of 2-5 years in a healthcare setting preferably LTC-related. Administrative, nursing, social work, or liaison experience preferred.
  • Associates degree in a health related or LPN, LCSW, or RN license
  • Fully vaccinated
  • Valid Driver’s License
  • Basic understanding of commercial insurances, Medicare, and Medicaid reimbursement. Advanced computer experience with Microsoft, Excel and clinical or sales software.

Job Type: Full-time

Pay: $33.00 per hour
