Duties: - Provide direct patient care in a Long Term Care facility setting. - Administer medications and treatments as prescribed by physicians. - Monitor and assess patient conditions, including vital signs, symptoms, and response to treatment. - Collaborate with interdisciplinary healthcare team members to develop and implement individualized care plans. - Provide health coaching and education to patients and their families. - Ensure infection control protocols are followed to maintain a safe healthcare environment.
Qualifications: - Valid Registered Nurse (RN) license in the state of Illinois. - Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
