Job Description:

We are actively seeking a compassionate and skilled Licensed Practical Nurse (LPN) with a passion for pediatric care to join our dynamic home health team. In this role, you will deliver personalized, comprehensive care to young patients in their homes, working collaboratively with family members and a multidisciplinary team to enhance patient well-being.

Responsibilities:

• Provide direct nursing care to pediatric patients in personal settings under the supervision of registered nurses or physicians.

• Conduct patient assessments, implement nursing interventions, and monitor health progress.

• Administer medications and treatments as prescribed, carefully monitoring for adverse reactions.

• Develop and implement tailored care plans in collaboration with healthcare professionals and families.

• Educate family members on managing their child’s health condition, promoting continuity of care at home.

• Ensure accurate documentation of patient care and progress through electronic medical records.

• Uphold the highest standards of patient safety, privacy, and comfort during home visits.

Qualifications:

• Valid and current LPN license in Tennessee, with a strong background or interest in pediatric nursing.

• Proven experience as an LPN, ideally in home health care settings or with pediatric patients.

• Excellent interpersonal skills to communicate effectively with children and their families.

• Strong organizational skills with the ability to manage multiple tasks simultaneously.

• Deeply compassionate and patient-focused, with a commitment to delivering empathetic and respectful care.

• Meticulous attention to detail, particularly important in administering treatments and medications to pediatric patients.

• Capable of working independently in a home care setting, while also being a collaborative team member.
