Job Summary:
Interim HealthCare, a leading home healthcare franchise, is seeking an experienced and compassionate RN Case Manager to join our team in Overland Park, KS. As an RN Case Manager for our home health division, you will work closely with patients and their families to provide comprehensive and high-quality nursing care in the comfort of their own homes. Responsibilities:
- Assess patients' conditions and develop individualized care plans in collaboration with physicians and other healthcare professionals to ensure the best possible outcomes
- Provide skilled nursing care and treatments to patients in accordance with physician orders and established standards of practice
- Educate patients and their families on their conditions and medications, and provide support and resources for managing their care at home
- Communicate regularly with the interdisciplinary team, including physicians, therapists, and social workers, to coordinate and facilitate comprehensive care
- Monitor patient progress and adjust care plans accordingly to ensure optimum recovery and wellness
- Document all patient care and interactions accurately and thoroughly in electronic medical records
- Maintain a current knowledge of home healthcare regulations and best practices
- Work independently and efficiently, adhering to established schedules and meeting productivity standards
Requirements:
- Current, unrestricted RN license in the state of Kansas
- Minimum of 2 years of clinical nursing experience, preferably in home health or a related setting
- Strong clinical and critical thinking skills to assess and manage complex patient cases
- Excellent communication and interpersonal skills, with the ability to effectively collaborate with a diverse team of healthcare professionals
- Proficiency in electronic medical records and other computer applications
- Must have reliable transportation and a valid driver's license
- Ability to provide excellent patient care in a variety of personal settings
- Must pass a background check and drug screen
