Job Summary:
Interim HealthCare, an industry-leading home healthcare and medical staffing agency, is seeking a skilled and compassionate Registered Nurse (RN) to join our team as Case Manager in Charlottesville, VA. This is a contracted position with a pay rate of $52 per hour with day shift availability You must have at least 2 years of recent experience.

Responsibilities:

- Develop, coordinate, and monitor client-specific healthcare plans
- Conduct initial and ongoing assessments of each client's healthcare needs
- Communicate with physicians, caregivers, and other healthcare professionals to ensure coordinated care
- Maintain detailed and accurate documentation of client care and progress
- Provide skilled nursing care, including wound care, medication administration, and other treatments
- Foster a compassionate and supportive relationship with clients and their families
- Advocate for clients' rights and promote self-advocacy
- Maintain compliance with all federal, state, and local regulations
- Participate in continuous education and professional development opportunities
Requirements:
- Active and unrestricted Registered Nurse (RN) license in the state of Virginia
- Experience in case management is preferred
- Excellent communication and interpersonal skills
- Strong organizational and time-management abilities
- Ability to work independently and as part of a team
- Must pass a comprehensive background check and drug screening
