POSITION : Intake Specialist Homelessness Prevention System (FT, Non-Exempt)
REPORTS TO : HPS Supervisor
COMPENSATION: $23.00/hr. to $26.00/hr. depending on skills and experiences


JOB SUMMARY:

ICAN, located in San Jose, CA, is looking for a bilingual and bicultural English/Spanish or English/Vietnamese Intake Specialist to support our HPS (Homelessness Prevention System) Program.
As the Intake Specialist, you are the first point of contact for potential clients who are seeking rental assistance and case management for our Homelessness Prevention System (HPS) program. You will guide potential clients through a screening process (triage) to determine eligibility for HPS, and if eligible, invite them to become a client by going through an overview of our program. You will assist clients in the completion of initial applications as well as addendums and updates for admission into the program. Once they are active and admitted into the program, you will then transfer the case over to the HPS Case Managers.
The ideal candidate must be: 1) able to speak and read English/Spanish or English/Vietnamese; 2) highly organized and detail oriented; 3) strong communication skills; 4) able to work in a fast-paced environment; 5) understanding of and sensitive to the needs of low-income families; and 6) able to work flexibly, creatively, and with a growth mindset;
ORGANIZATIONAL BACKGROUND:
ICAN was founded in 2000 by three refugees of the Vietnam War with the mission to lend a helping hand to the families, friends, neighbors and fellow countrymen whom they left behind. While still maintaining our commitment to our charitable projects in Vietnam, ICAN has evolved throughout the years into an organization that supports and strengthens the local Vietnamese community in the Bay Area. Our US-based programs include positive parenting workshops, baby care classes, and kindergarten readiness programs. We also have community support programs such as: Women support group, Homelessness Prevention Program, Digital Literacy, Youth Financial Literacy, to name a few. Learn more about us at www.ican2.org
The mission of ICAN is to inform, engage, and inspire Vietnamese Americans to help raise the next generation of caring leaders through humanitarian programs, culturally responsive social programs, and communitybased research.


ESSENTIAL DUTIES & RESPONSIBILITIES:
1) Outreach and Engagement: (5%)
a) Conducts client outreach at various community events
b) Establishes and maintains relationships with other community support agencies to network client services
c) Attends all required agency, staff, and community-based meetings and trainings as needed

2)
Prescreening and Assessment: (40%)
a) Take calls, emails, and referrals to determine eligibility into the HPS program
b) Conduct phone screenings with families to assess their need for Homelessness Prevention services.
c) Enter prospective clients into our database to capture data and for case follow up
d) Determine triage and urgency of the case for HPAT assessment. Assists families with immediate crisis prevention, advocacy, problem-solving and other appropriate interventions, as needed
e) Conducts HPAT interview and assessment to determine eligibility of HPS program and enter result of the HPAT score into the HMIS database.
f) If client is eligible, then start client in the HMIS system and ICANs client folder
g) If client is ineligible, then refer them to EAN partners


3)
Intake of Eligibility Documents: (40%)
a) Collects and compiles necessary documents to verify eligibility and needs of client
b) Verify eligibility documents for completeness and validity
c) Explain program guidelines and eligibility criteria to clients
d) Storing and uploading clients information into secured folders to ensure clients privacy of information
e) Preparing clients folder for completeness and prepare Case Manager to take over the case.
f) Stay informed of community resources available for HPS clients

4)
Data Management and Quarterly Reporting (10%)
a) Maintain accurate and keep confidential client records
b) Maintains case files and progress notes, adhering to professional standards and designated time frames
c) Maintains accurate, updated, confidential case files on ICANs secured database
d) Documents program activity, including prescreens, intakes, financial assistance, and case management into HMIS within one to 3 business day of activity
e) Submits quarterly narrative, data and other reports as needed
5) Others (5%)
a) A commitment to uphold ICANs Vision, Mission, and Values.
b) Adhere to organizational policies and procedures.
c) Participate in the continuous improvement of ICAN and its 5 Year Strategic Plan.
d) Perform other duties as assigned
REQUIRED EDUCATION & SKILLS :
Minimum 1 year of experience in social services or a related field
Strong organizational skills; ability to plan, organize, prioritize and multi-task.
Ability to work with little direct supervision.
Strong communication skills, including the ability to deliver difficult messages with compassion and to communicate respectfully with families in stressful situations.
Ability to thrive in a flexible, fast-paced environment, while maintaining a positive, solution-oriented approach
Computer skills including proficiency in MS Word, Excel, PowerPoint, Google Suite, Adobe, and the ability to quickly learn and develop proficiency with tracking database systems
PREFERRED KNOWLEDGE & ABILITIES :
Bilingual and bicultural in English/Spanish or English/Vietnamese. English/Spanish or English/Vietnamese speaker, with ability to read and write in English/Spanish or English/Vietnamese.
Four-year degree in social work preferred or related degree with case management experience.
Candidate should be passionate and have commitment to building communities and promoting civic engagement.
Candidate should have a willingness to learn and a growth mindset.
Experience working with persons from diverse ethnic and cultural backgrounds who are economically disadvantaged.
OTHER REQUIREMENTS :
  • Must be able to lift 25 pounds as the service is sometimes physical. For example, setting up for events which may require transporting or lifting equipment and materials, e.g., easels, flip charts, refreshments, brochures, chairs, tables, etc.
  • Must be able to report to service site on a regular and reliable basis, and able to travel between sites and to attend offsite events throughout the county, as appropriate
  • Must be able to work a flexible schedule, which includes evenings, weekends, and holiday hours, as appropriate, to coordinate and provide services and activities for families during non-traditional hours.

LICENSES, CERTIFICATIONS, REGISTRATIONS :
Must possess a valid California Drivers license, reliable transportation and current auto insurance
OTHER QUALIFICATIONS :
  • Must be at least 18 years of age with permit to work in the US
  • Clearance of fingerprint background check
  • Must have valid TB screening and COVID19 Vaccination
