PURPOSE OF POSITION

The Value-Based Care Consultant will be primarily responsible for providing support to the strategic initiatives and value-based contracts of IowaHealth , a clinically and financially integrated network of Federally Qualified Health Centers in Iowa. This position will be responsible for working in partnership with key staff at member health centers to take actions to propel success and ensure that annual performance targets are met.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The following duties are normal for this position. These are not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

· Work with the Director of Value-Based Care and the Senior Director of Transformation to understand, precisely document, and prioritize all value-based contract requirements, and then organize these requirements into a coherent roadmap that outlines key activities and milestones.

· Monitor and maintain roadmap and progress against key activities and compliance with all contract requirements.

· Lead the planning, facilitation, and follow-up for the IowaHealth Clinical Quality Committee (CQC), Consumer Advisory Committee (CAC), IH Finance Committee and Value-Based Care Task Force (VBCTF).

· Use available data and best practices to develop creative and innovative strategies, interventions, and other performance improvement strategies in collaboration with health center members.

· Assist IowaHealth member health centers in developing Models of Care to drive to meet identified performance outcomes.

· Work with the Data and Technology team to help member health center care teams access and understand data and reports to define and implement new population health management actions or interventions.

· Provide thorough written summaries, status updates, assessments, and scorecards in collaboration with data team members as needed.

· Provide organizational support for IowaHealth including: Ensuring documents and other collateral are stored, and that reporting requirements are met; Assists with data gathering & collateral development for quarterly IH participation standard scorecard. Lead the coordination, providing content, and facilitating in-person Transformation Collaborative events, which occur tri-annually, as needed.

· Track payer payments and escalate issues as necessary to the Director.

· Prepare and present progress against goals monthly to Boards, Members, Committees, or health center executive teams as directed.

· Assist with the monitoring of and strategy creation for contract agreements with payers in conjunction with the Director of Value Based Care.

· Perform other duties and projects as assigned, exhibiting willingness to adapt when requested to get the job done.

Telecommuting and Travel:

This position is required to be on-site at least two days per work week.Some travel may be required to community health centers via vehicle or conferences via air travel.

QUALIFICATIONS

Education and/or Experience:

· Bachelor’s degree required; Degree in health care administration, nursing, business management, public health, or another health-related field preferred. A person not possessing a bachelor's degree may be considered if their combination of education and experience matches what is needed for this role.

· At least three years of professional experience working in a health care setting or organization.

· Experience working in an accountable care organization preferred.

· Experience working with health care analytics and performance reporting.

· Experience with healthcare transformation, process improvement, workflow redesign, and continuous quality improvement methodologies preferred.

· Excellent administrative skills required to manage work on a variety of programs with varying deadlines and program responsibilities.

· Must possess strong communication skills – including public speaking, meeting facilitation, and writing reports and other documents.

· Ability to communicate effectively with health center staff, clinicians, community leaders, and government officials.

· Ability to work at a fast pace, take initiative, problem-solve, and develop high-quality content.

· Ability to manage multiple priorities.

· Must be flexible, self-motivated and a team-player.

Job Type: Full-time

Pay: $61,000.00 - $77,000.00 per year
