IRMC is Hiring an After Care Coordinator - IRMC Physician Group Social Services - Full Time Near Indiana, PA

Qualifications:

  • Candidate must complete at minimum a qualifying LPN program, RN preferred. Knowledge of case management philosophies and principles beneficial.
  • The ability to triage patients based on their medical need and IPG protocols
  • Possess knowledge of public and private social services providers within the community and programs available.
  • Communicate exceptionally well orally, in writing, and over the telephone.
  • Use initiative and independent judgment; organize own work, set priorities, and meet critical deadlines.
  • Understanding concepts of case management: navigating healthcare system.

Responsibilities:

  • Responsible for calling patients daily who visit the ER and DC from inpatient services.
  • Track and case manage repeat patients.
  • Address noncompliance within patient care.
  • Track appointment history, follow up with repeat no shows.
  • Scheduling patients on the continuum of care.
  • Complete and document for transitional care, medication checks, and chronic care management for patients.
  • Coordinate care with inpatient departments to ensure quality care and post discharge follow up.
  • Educate patients on indigent programs for medication, equipment, and services for uninsured and underinsure patients.
  • Provide ongoing case management services by phone and/or face-to-face; track client progress and well-being, evaluate treatment effectiveness, while making adjustments based on need to assigned caseload.
  • Prepare complete and accurate progress notes; write correspondence, and other written materials
  • Utilizes a team approach, confer with treatment team to provide quality care to patients.
  • Assist in maintaining office responsibilities including but not limited to, faxing, copying, and answering phones.
  • Manage basic Excel databases, inputting, and organizing data into visual presentations to administration for AfterCare outcomes.
  • Assist with community outreach and events as needed.
