• Graduated from an accredited Occupational Therapy (OT) college or university
• Unencumbered Occupational Therapy (OTR/L) license for New York
• Conduct home visit Occupational Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation
• Proficient in EMR documentation (we use Hello Note but experience in this system is not required)
• CPR Certification
