JAMHI Health & Wellness, Inc. is looking for a Case Manager to join our Community Housing team!

The Community Housing Case Manager is an important part of the support system of our Community Housing clients. The ideal candidate should be comfortable with a busy schedule both in and out of the homes. They will accompany clients to their community appointments and conduct daily documentation and collaborative treatment plan reviews. They will display patience, respect, and courtesy when assisting residents in their daily living routines, and when asked to assist with group activities.

This is a full-time, permanent position with working hours of Monday through Friday from 8 a.m. until 4:30 p.m. Salary placement is based on experience, and demonstrated skill sets

You Have…

Excellent interpersonal communication

Time management, planning, and organization

Experience with Microsoft Office Suite of programs

1 year of direct service experience
