Description
You will be responsible for managing high-quality Perinatal Case Management (PCM) to clients that support, enhance, and coordinate the health and other services recommended for mothers and babies. The role includes assessment, goal setting, care coordination, crisis management, coordinating process improvements and executing weekly standing tasks. You'll establish and maintain strong working relationships with peers and partners and provide regular status updates, summarizing issues to key points and actions required. The Case Manager is expected to demonstrate exceptional flexibility, willingness to learn, ability to work effectively on a team, empathic and strengths-based skills, as well as exceptional time management skills. Finally, you'll represent the organization to other internal groups and clients to ensure that Janus’s objectives are well understood.
Pay & Benefits
At Janus, base salary is one part of our total compensation package and is determined within a range. This provides the opportunity to progress as you grow and develop within a role. The base pay range for this role is between $25.00 and $28.00 per hour and your base pay will be considered on your job-related skills, training, qualifications, experience, and relevant education.
