Position Summary: The Service Providers (SPs) are primarily responsible for delivering quality Children & Family Treatment & Support Services (CFTSS) and/or Home and Community Based Services (HCBS) services to eligible youth to support the achievement of safety, well-being and permanency objectives.
CFTSS and HCBS are Medicaid funded services that are designed to allow children/youth ages 0-21 who are at risk of developing or have been diagnosed with behavioral, developmental or medical diagnoses to participate in a vast array of developmentally and culturally appropriate services in the home and in the community.
