Job Summary:
- Must have an associate's degree, preferably a bachelor's degree, in social work or related field
- 1-3 years of experience in a hospital setting, skilled nursing facility, or health/community agency is required
- Must hold a Social Workers MA State SW licensure, including LCSW, LSW, LSWA and/or LICSW
- Ability to work independently and in a team setting
- Excellent verbal and written communication skills
- Experience with computers, specifically for data collection, report writing, and quality monitoring
- Ability to work with community agencies to mobilize resources
- Demonstrates flexibility and adaptability to change
- Physical demands include exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently. Frequently reaching and handling objects is required.
