The HUD Supportive Housing Case Manager position is responsible for the provision of an array of mental health services to individuals living with serious mental ill and experiencing homelessness based on the HUD definition of homeless residing in supervised independent living apartments purchased through the federal HUD Supportive Housing Grant.

Division: Outreach Supported Housing
Normal Work Hours/Days: 40 hours per week - Monday through Friday.
Flexibility required for varied work hours based on program need and individual client
requirements. Some evening and weekend hours work will be required.
Primary Job Functions: Knowledge of the rules and regulations governing the human series provider agencies in the catchment area.
Provide access to a wide range of services to seriously mentally ill individuals who reside in the community.
Responsible for providing direct assistance in the maintenance of apartment living. Act as a liaison between the
tenant, lease holder and apartment complex. Assist in making sure the apartment is well maintained through monthly
inspections.
Responsible for providing necessary transportation to clients.
Responsible for assessing individual needs and devising individualized needs assessment and case plan.
Responsible for developing and adequately maintaining detailed and accurate client records.
Any and all other duties as assigned by supervisor.
Minimum Qualifications:
Bachelor of Arts or Bachelor of Science in a human service related field. Two years of experience working with
individuals living with mental illness.
Valid Alabama driver’s license and driving record acceptable to agency automobile insurance carrier for inclusion on
agency policy. Suitable automobile to permit the required travel. Must show maintain car insurance equal to or in
excess of the State of Alabama standards for automobile insurance coverage.
This position requires completion of the State of Alabama Adult Serious Mental Illness case management training.
Knowledge, Skills, Abilities:
Knowledge of mental illness, symptoms and treatment regimens.
Ability to work autonomously
Knowledge of the special needs and behavioral characteristics of individuals experiencing serious mental illness.
Knowledge of the community and available community resources.
Knowledge of the legal and ethical issues relative to confidentiality of client records and the ability to interpret and
apply them within the scope of the position.
Ability to communicate effectively with a multi-disciplinary team.
Working knowledge of developing needs assessments and case plans.
Ability to document activities and maintain records in a timely fashion.
Skills in interpersonal relationships and dynamics.
Considerable written and expressive communication skills.
Acquainted with the use and side effects of psychotropic medications.
Ability to lift, bend, and move objects on a day-to-day basis.Qualifications
