The Multidisciplinary Program Manager should be a passionate, innovative, driven, technically capable, problem-solver who will lead highly visible, multidisciplinary projects/programs within Jefferson’s Comprehensive Parkinson’s Disease Center. The program manager will facilitate and manage aspects of the multi-disciplinary program in primarily the outpatient setting. This position requires strong interpersonal relationships with patients, physicians, care team partners to assist patients in navigating the Physical therapy services and Deep Brain Stimulation (DBS) pathway. It involves coordinating essential evaluations for patients, including diagnostic procedures and clinic visits; and the scheduling and facilitation of these. The ability to navigate the Jefferson medical system will be needed to achieve these aims.

The Multidisciplinary Program Manager will be tasked with the navigation and further program development for the Deep Brain Stimulation Program and the Rehabilitation Programs within the Jefferson Comprehensive Parkinson’s Disease Center (JCPDC)

The Program Manager is committed to superior service and effective, proactive communication, both internally with peer staff and externally with other Departments. The Program Manager takes ownership of problems, provides leadership in developing solutions, and is committed to collaborative cooperation with fellow team members.

The Program Manager works within a flexible, self-directed, team approach in which initiative and timely follow-through are valued. A positive, service-oriented, demeanor and outlook that puts the customer first is a requirement of this support position.

ESSENTIAL FUNCTIONS:

Navigation through Jefferson’s Deep Brain Stimulation (DBS) pathway:

  • Demonstrates an ability to assess patient needs and make appropriate referrals to provide navigation and guidance to Movement Disorders patients through the Deep Brain Stimulation (DBS) pathway.
  • Navigation services may include addressing barriers, resource referral, and care coordination.
  • Utilizes and identifies available resources to assist patients with overcoming barriers to receiving appropriate access to care and clinical trials.
  • Employs seamless processes for patient referrals to navigation services across the DBS continuum.
  • Facilitates visits necessary for the pre and post operative evaluation through neurosurgery, neurology, neuropsychology and radiology in patients undergoing DBS surgery.

Development of Multidisciplinary Rehabilitation Programs through the JCPDC

  • Establish patient referral process for Physical, Occupational, and Speech therapy that maintains continuity of care, utilizes best practices.
  • Development of novel and innovative exercise and rehabilitation programs and classes
  • Coordinating various rehabilitation programs within Jefferson (i.e. TJUH, Magee, Moss, Jefferson NJ) to create a unified comprehensive approach to therapy for the PD patient population.
  • Lead and manage classes and specialized evaluations of PD patients of the JCPDC either at the Honickman Center, the Navy Yard, or at other sites to be determined.
  • Navigates the various community programs and resources available in the community.

Education and Research:

  • Coordinates the development, utilization, and evaluation of appropriate educational resources and interventions that assist the patient/caregiver in developing the skills for self-care.
  • Assists in the coordination and implementation of educational workshops to inform the community of resources offered by the JCPDC.
  • Assesses the educational needs of the patient/caregiver.
  • Utilizes clinical research to support evidence-based clinical decision making and practice.
  • Uses advanced subject matter expertise in the therapeutic area or clinical research to solve complex problems or foster innovation.
  • Maintains a services database that captures how patients were referred, the types of barriers patients faced, how the patient interacted with our team, who provided the service and how the multidisciplinary team was informed of the patient's needs.
  • Oversees implementation of data quality processes, data flow plans, and new technologies
  • Participates in quality/performance improvement programs and projects, product evaluation, outcomes evaluation studies, and/or clinical research.

Organization and Logistics:

  • Develops strategies to improve or maintain the effectiveness of multidisciplinary programs.
  • Keeps current with advances in the scientific area, considers the impact on the DBS program, and oversees necessary implementation.
  • Establishes and monitors processes to improve patient satisfaction.
  • Establishes policies and procedures in conjunction with supervisors to ensure consistency and standards for the program.
  • Interacts with multiple departments and staff to provide safe, efficient care.
  • Maintains confidentiality and compliance with HIPAA, Medicare and other government regulations.
  • Participates in the organization, development, and presentation of programs regionally and nationally.
  • Ensures the implementation of strategic business plans that aligns with Jefferson and the JCPDC’s vision, strategic plans, and goals and monitors performance.
  • Collaborates with Jefferson Marketing team, JCPDC as well as Neurosurgery and Rehabilitation leadership to develop marketing and communication tactics that support strategies, direction, and increase reputation in the community.

Communication:

  • Establishes and maintains internal and external communications to ensure successful partnerships.
  • Communicates effectively with others, regardless of reporting relationship, to accomplish shared work objectives.

EDUCATIONAL/TRAINING REQUIREMENTS:

  • BA/BS in Business, Health or related field.
  • Advanced degree preferred.

CERTIFICATES, LICENSES, AND REGISTRATION:

  • Project Management Institute (PMI) Certification or Project Management Professional (PMP) is preferred.

  • LEAN/Six Sigma certification preferred.

EXPERIENCE REQUIREMENTS:

  • Excellent communication and interpersonal skills with strong leadership and multitasking abilities.
  • Experience managing large, complex projects.
  • Clinical and/or administrative experience with patients diagnosed with Parkinson’s Disease and related disorders.
