Case Manager RNs are responsible for the following:
- Advocates for the patient and family throughout the entire episode of care.
- Monitors patient’s progression intervening as needed to ensure efficient patient progress.
- Coordinates all aspects of discharge planning and establishes a safe discharge plan.
QualificationsBachelors of Nursing required
Three (3) years case management/discharge planning experience
CCM certification preferred
Active RN license in NJ/PA
