The Emergency Housing Case Manager in the Emergency Response Program plays a crucial role in supporting the Housing and Integration Specialist conduct case management for client in the emergency sheltering system.

Responsibilities:

• Provide transportation for clients to various appointments, including medical, educational, and social service appointments.
• Conduct intake assessments to determine clients' housing needs.
• Develop individualized housing plans in collaboration with clients, identifying short-term and long-term goals for housing stability and self-sufficiency.
• Assist clients in accessing emergency housing resources, including shelters, transitional housing programs, and rapid rehousing assistance.
• Provide ongoing case management support to clients, including advocacy, crisis intervention, and assistance with navigating social service systems.
• Coordinate with internal and external partners to address clients' needs comprehensively, including collaborating with healthcare providers, mental health agencies, substance abuse treatment centers, and legal services.
• Monitor clients' progress towards housing goals, conducting regular check-ins and reassessments as needed.
• Assist clients in obtaining necessary documentation and completing housing applications, rental agreements, and other paperwork.
• Provide education and support to clients on topics such as budgeting, tenant rights, and maintaining housing stability.
• Maintain accurate and up-to-date client records, including case notes, service plans, and progress reports.
• Participate in staff meetings, trainings, and professional development activities to enhance knowledge and skills related to emergency housing and case management.

Qualifications:

• High school diploma or equivalent; Bachelor’s degree in related field preferred.
• Fluent in English and proficiency in at least one other language (Haitian Creole or Spanish Preferred).
• Experience working with diverse populations, particularly refugees or immigrants, and sensitivity to cultural differences.
• Excellent communication skills, both verbal and written.
• Strong organizational skills with the ability to prioritize tasks and manage time effectively.
• Valid driver's license with a clean driving record and access to a reliable vehicle.
• Ability to maintain confidentiality and handle sensitive information with professionalism and discretion.
• Proficiency in basic computer applications and willingness to learn new software programs.
• Flexibility to adapt to changing program needs and work occasional evenings or weekends, as required.

Job Type: Full-time

Pay: $21.00 - $23.00 per hour

Expected hours: 37.5 per week
