Overview
We are seeking a dedicated and experienced Service Supervisor to lead their own respective team. A Service Supervisor must be able to implement individualized plans that reflect person-centered choices and self-determination concepts for participants in the Adult Day Health (ADH) community-based program and Workshop program. The Service Supervisor (SS) reports directly to the Administrator and communicate with DOH-DDD affiliations and legal entities. Specific responsibilities are as follows:

Duties/Skills:

  • Assure clients rights are protected.
  • Notifies and documents any concerns or problems regarding the participant's physical, mental, social status, and interests.
  • Oversee the implementation of customer service policies and procedures to enhance efficiency.
  • Must be able to read, prepare reports, and record daily pertinent information on participant's progress and attendance. Be able to thoroughly complete appropriate reports (In-House Adverse Reports and/or DOH-DDD Incident & Adverse Event Reports, Interdisciplinary Entry, and Communication Book & Data Flow Sheets, etc.)
  • Must be able to develop and provide mandatory DOH-DDD reports defined in the current Waiver Standards to designated case managers, guardians, and other legal entities relating to each participant in the facility. Designated DOH-DDD reports include Quarterly Reports, IP Methods, Interest Inventory, etc.
  • Must maintain communications with DOH-DDD stakeholders and legal affiliations and be compliant to each entity's regulations and Medicaid Waiver Standards.
  • Perform monthly on-site observations and reports evaluating the DSW and their methods in implementing specific IP objectives for each of their assigned participants per service (ADH, CLS, PAB, and RESPITE).
  • Must comply with policies and procedures outlined in the Employee Handbook (sick leave policy, swapping shifts, dress codes, etc.) and in the DOH-DDD Medicaid Waiver Standards.
  • Be familiarized with assigned participant's medical history and Individualized Plan (IP). Must be able to implement IP accordingly and adhere to confidentiality of participant's information.
  • Be familiarized with assigned participant's caregiver, case manager, guardian, or other members of participant's Circle of Friends (CF).
  • Attends and lead in staff meetings, IP conferences, and scheduled staff training sessions.
  • Provides team support in implementing center-based and community-based activities.
  • Must possess knowledge and skills necessary to provide appropriate care to clients with intellectual and developmental disabilities.
  • Must be able to calmly resolve problems, handle conflicts and make effective decisions under pressure. Must be mentally adaptable and flexible in leading with a variety of people. Possesses the abilities to give, receive, and analyze information, formulate work plans, and prepare written materials, and articulate goals and action plans. Personal maturity is an important attribute.
  • Must ensure in health and safety of participants and staff within their surroundings.
  • Assist (if needed) feeding, teeth brushing, dressing, grooming, changing diapers, and bathing of incontinent participants, assigned to the employee.
  • With assistance, may require lifting and carrying participants from sitting to standing position, and assisting in their movements or other ambulatory duties.
  • Must be ready to administer First Aid and Cardiopulmonary Resuscitation (CPR) and be able to secure an ambulance if needed.
  • Must be able to sit, stand, stoop, bend, push, or lift participant and equipment if needed. Requires the manipulation of objects, such as grasping with hand/wrists, reaching for objects above should height or other activities depending on the environment where training is taking place.
  • Must be able to communicate verbally and in writing with members of the team and other disciplines. Must have compassion and tolerance to work with participants, staff, administrations, and others to ensure that participants' best interests are first taken into considerations.
  • Must ensure sufficient amount employees are supervising participants, in accordance with the maximum staffing ratio provided by State of Hawaii, Department of Health Developmental Disabilities Division.
  • Effectively communicate with other employees regarding supervision of their designated participant(s) during breaks, ISP conferences, Community-Learning Service-Group (CLS-Group), or other activities that may leave participant(s) unattended. Supervision of participant(s) must always be ensured at all times, either from DSW employees or administrative staff.

Job Type: Full-time

Pay: $20.00 - $25.00 per hour

Expected hours: 30 – 40 per week
