We are seeking a highly competent and dedicated Permanent Resident Care Manager (RCM) to join our dynamic healthcare team.
The successful candidate will be responsible for managing the overall healthcare needs of our residents, ensuring their health and comfort.
This role demands a compassionate professional with an excellent understanding of resident care management and a strong commitment to improving the quality of life for our residents.
The ideal candidate will be a critical thinker with exceptional organizational and leadership skills, and a passion for delivering top-quality patient care.
Responsibilities :
1. Develop, implement, and review comprehensive care plans for each resident, based on their unique healthcare needs and preferences.
2. Coordinate and oversee the work of the healthcare team to ensure that all care plans are executed effectively.
3. Conduct regular assessments of each resident's health status, making necessary adjustments to their care plans as required.
4. Maintain accurate and up-to-date records of each resident's care, including their medical history, treatments, and progress.
5. Establish and maintain effective communication with residents, their families, and healthcare team members to ensure that all parties are well-informed about the resident's health status and care.
6. Advocate for residents' health and wellbeing, ensuring that their needs and preferences are respected and met.
7. Collaborate with other healthcare professionals and departments to ensure the provision of comprehensive, integrated care.
8. Comply with all relevant regulations, standards, and policies relating to resident care management.
9. Provide leadership and guidance to the healthcare team, fostering a positive and supportive work environment.
10. Participate in ongoing training and professional development activities to keep up-to-date with the latest advances in resident care management.
Qualifications :
1. Bachelor's degree in Nursing, Healthcare Administration, or a related field. A Master's degree will be an added advantage.
2. A minimum of 5 years' experience in resident care management, preferably in a long-term care setting.
3. Proven experience in developing and implementing effective care plans.
4. Excellent knowledge of resident care management principles and best practices.
5. Exceptional leadership and organizational skills, with the ability to coordinate and oversee the work of a healthcare team.
6. Outstanding communication and interpersonal skills, with the ability to establish and maintain effective relationships with residents, their families, and team members.
7. Strong advocacy skills, with a commitment to promoting residents' health and wellbeing.
8. Proficiency in using healthcare information systems and software.
9. A current, valid license to practice as a Registered Nurse or equivalent.
10. A commitment to ongoing professional development and learning.
