Responsibilities:
Provides comprehensive case management and care coordination for a diverse panel of patients with identified needs spanning age, condition, and population variables
"Coaches" (rather than "teaches") patients to improve their health behavior to attain health-related goals, while working as part of a health care team (Providers, Nurses, Social Workers, Patients, Family, Caregivers, etc.)

Education:
Graduate from an accredited school of nursing
Active, unrestricted licensure as a Registered Nurse in a state
Current state's driver's license with the ability to travel to hospitals, skilled nursing facilities, patient's homes, and other sites where patients receive care as needed
Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements.

Preferred Qualifications:
Certification in Geriatric Nursing or Geriatric Case Management
5 years of nursing experience, preferably with older patients
Proficient in computer use, the Internet and health information technology
Demonstrated excellent communication, interpersonal and organizational skills, with a flexible and creative approach to problem-solving.
Proven ability to work effectively both independently as well as a member of an interdisciplinary team.
