Johns Hopkins Medical Management Corporation is Hiring a Home Care Coordinator Assistant Near Baltimore, MD

Overview

Johns Hopkins Intrastaff is the internal staffing agency for the Johns Hopkins Health System and partner hospitals, providing temporary support to a variety of the Johns Hopkins locations. Monday - Friday

8:30am-5:00pm

Responsibilities

  • Effectively and efficiently coordinates referral to JHHCG and outside agencies.
  • Services requested are arranged; alerts of delays.
  • Responds to all customer (internal & external) requests in a timely manner.
  • Maintains records of HCC functions.
  • Maintains updated master resource manual for office including accurate agency lists and ensures HCC has inventory.
  • Files log sheets and appropriate clinical documentation so that information is retrievable and well‐organized.
  • Reviews referral packet to ensure all required information is present.
  • At the end of each day, ensures all paper is filed and office is neat and professional.
  • Inputs HCC information into HCC flow sheet accurately and timely.
  • Assists with scheduling and facilitating Homecare 101s for assigned HCCs
  • Completes time studies completely and timely.
  • Maintains office in a professional and efficient manner.
  • Phones answered within three rings using standard greeting; always offer to maintain inventory of supplies for office operations and HCC staff.
  • Maintains positive relationships with unit staff and outside agencies.
  • Verifies insurance and provides needed information to agency and other patient information.
  • Effectively communicates with agency (verbally and electronically) to ensure all information is provided to safely initiate care (referral, attachments, medication list, physician orders, and discharge instructions) and confirm receipt of referral with receiving agency.
  • Ensures patient discharge occurred and if not, follows up with agency for delays or changes. Provides agency any necessary follow‐up to ensure patient care is provided as anticipated. Notifies HCC & team once arrangement confirmed.

Qualifications

High school diploma or GED required, some college preferred

• One to two years data entry experience required• One to two years customer service experience in a healthcare setting
