Location:

Johns Hopkins Hospital located in Baltimore, M.D.

Schedule:

Mon-Fri.
8:30 AM - 5:00 PM.

Pay Rate:

$42.00 - $46.00 per hour (Based on location and experience).

Responsibilities:

Provides direct psychological, social, and practical care to a select caseload of patients and families including social work assessments, problem identification, goal setting, and implementation of care plans.
Makes referrals to outside agencies and coordinates continuity of care.
Works collaboratively with and provides leadership with the multidisciplinary team to optimize patient outcomes.
Serves as a clinical resource expert in a specialty area.
Focuses on the interaction between the social worker, the patient family, and the environment of care.
Demonstrates expertise in diagnosis and clinical interventions.
Apply social work knowledge and skills in the delivery of patient care.
Provides and coordinates direct care to patients and families.
Develops a comprehensive biopsychosocial assessment.
Uses each specific knowledge to identify and implement a treatment intervention.
Employs appropriate treatment modality with individuals, couples, families, and/or groups.
Uses self effectively to engage, collaborate, advocate, and problem solve while developing an effective working relationship with patient and family.
Enables patients to become empowered in their own treatment and care.
Formulates a clear description of the problems, goals, and interventions.
Timely completion of medical records according to departmental and regulatory standards.
Documentation reflects appreciation and understanding of patients perceptual, cognitive, and emotional frame of reference. Identifies and demonstrates differing professional functions articulate social work role to the multidisciplinary team members.
Maintains a caseload appropriate to the patient population of the assigned work area.
May be expected to provide on-call coverage as needed.

Qualifications:

Masters degree from an accredited school of social work is required.
Current state of Maryland licensure at the LCSW level.
Two years or more of licensed clinical social work in a health care setting is required. Internship may be considered.
Knowledge of biopsychosocial and cultural elements of clinical social work practice.
Knowledge of procedures, systems, and organizational objectives used within social work.
Effective knowledge of community resources.
Knowledge of current healthcare environment.
Knowledge of an ability to gather relevant data to synthesize and summarize information and making judgments regarding patient care.
